Click Eligibility
Allow you enter your Aadhaar-linked mobile number or SECC name.
Once you submit, one time password will be sent to you.

List of Documents
If you are eligible for the scheme, then ensure that you have all of the following documents ready for application:
Aadhaar Card: As proof of your identity.
Ration Card: Only for the purpose of including details about your family members. The government scheme would be applicable for all members listed on the Ration card.
Other ID Proof: In case one fails to provide an Aadhaar card, then they can submit other official ID proofs issued by the government, and that are Voters ID, PAN card, or photographs of a passport.
How to Apply
Eligible beneficiaries must visit the nearest accessible Common Service Centre. All persons operating Common Service Centre centers are trained for registration processes. To find your nearest CSC, you may please refer to the official CSC locator on the PM-JAY portal .
At your CSC you will get your eligibility verification done by the operator. The application process will also be assisted by the operator.

Check Your Application
Once you have filled in the AB PM-JAY application, check on all your personal details again. Verify that the same name as issued in Aadhaar or the Ration Card has been selected, also confirm that your spouse’s name, children, and residential address are all correct. Following this, your application will be submitted online by the operator.
Login to AB PM-JAY eCard
When you successfully submit your application, the details will be verified. In case that is proper, the CSC operator will issue an e-card associated with Ayushman Bharat, which can be downloaded. The card is called your health card and will admit you free of charge to all hospitals. So, you have to keep it safe, because all benefits under the scheme will require that card.
Find a Hospital
Once you get Ayushman Bharat e-card in hand, you can go to any of the empaneled hospitals for free treatment. The PM-JAY website has a list of the hospitals that form a part of the scheme. You can locate your closest one on the official hospital finder site: hospitals.pmjay.gov.in by selecting your state and district for viewing a list of hospitals where you can avail free treatment under your AB PM-JAY card.
